Information needed to register your student(s) for the 2023-2024 school year.

A new student registration means you DO NOT have any children currently enrolled in the United Community School District. The registration process must be completed for all students before they may attend school. A student is not considered fully registered to attend school until all of the steps listed below have been completed. It is recommended you complete the registration process on a laptop or desktop computer, as the process can be difficult on a mobile device. Step 1: REGISTRATION Complete the online registration application HERE. If you have any questions, please contact Karla Keigley or Ann Howe-Gorden. Provide proof of the student’s age: copy of a birth certificate, US passport, adoption papers, or custody document.* Provide an up-to-date immunization record.* Provide proof of physical or well-child visit (ONLY required for PRESCHOOLERS) to the school nurse. Provide proof of residency: utility bill, bank record, lease or mortgage document.* Once your application has been approved, you will receive notification via email. Step 2: NEXT STEP After you receive notification of your application approval, login to the Infinite Campus Parent Portal using the information provided to you via email. Select “*More” from the left navigation menu. Click on "Online Registration" Choose "New Student" Complete all forms. Be sure to click "Submit" on all forms. Step 3: OTHER REQUIRED REGISTRATION FORMS* (NOT found in Infinite Campus...click on the links below) Complete the Supplemental Registration Form. * Complete the Medication Form. * Complete the Transportation Form. * Complete the Home Language Survey (must be printed and a hard copy turned in/sent to the school). * Certificate of Vision Screening (ONLY required for Kindergarten and 3rd grade students). * Certificate of Dental Screening (ONLY required for Kindergarten students). * Complete the Confidential Information Form. * Free and Reduced School Meal information (not required but encouraged). Parent Letter Instructions Application Waiver Form The additional documentation can be attached to an email and sent to Karla Keigley, can be a hard copy that is either mailed to the school (1284 U Ave, Boone, Iowa, 50036) or dropped off at the school main office, or can be attached in the online registration process. Step 3: FEES* No fees are due at this time for the upcoming school year. Free and Reduced meal applications will be available during July for the 2023-2024 school year. Fees will be assessed next fall. Please take the time to review all information and all forms to ensure your child’s registration is complete.
